If your request is for a third party form to be completed, this will need to be submitted to the surgery along with evidence of your consent to share information.
Letters and reports are not part of our NHS service and therefore are done in our own time and will a incur a non-NHS charge. The charge depends on the time required to prepare the letter. For further details of NHS service charges, please check our Practice Policies or contact the surgery.
The turnaround time for a letter request is up to 21 days.
