You must live within our catchment area to be able to register as a patient of Streatham Park Surgery.
You will need to make a registration appointment so that we are able to take your medical history, and you will also undergo a basic health check. Find out more about what you need to do in preparation for your registration appointment here.
If you need medical attention before your registration, we can see you for Immediate Necessary Treatment, however, a registration appointment is still required.
