What You Need to Do
Before we can offer you a registration appointment, we need to check your eligibility. In order to do this, we will need to see:
- Your passport or birth certificate
- 2 proofs of address — utility or telephone bills, bank statements (from within the last 3 months), tenancy or mortgage agreement
- Your previous address and postcode
- Details of your previous GP
Once your eligibility to register has been satisfied, you will be:
- Offered an appointment
- Asked to complete a registration slip
- Given a registration pack from the receptionist
- Advised that all the documents in the pack need to be completed prior to attending the appointment
- Asked to attend at least 10 minutes before your appointment time for administrative purposes
- Asked to provide us with a record of previous vaccinations
- Asked to bring a record of any medications you are taking regularly
- If you are registering children under the age of 5, please bring in your child’s Red Book, or a record of all their immunisations. Please note that, if you do not bring this information to the appointment, the registration process cannot be completed
Note: If you do not attend your registration appointment, and do not cancel, this may affect your chances of registering with the practice.
