At Your Registration Appointment

You will be:

• Asked to provide a sample of urine
• Given a registration health check, which will include measurement of your height, weight and blood pressure
• The health care assistant will go through your questionnaire and take a history
• As part of the health check, you will be offered screening and advice for alcohol, smoking and Chlamydia

Note: Routine prescriptions cannot be given at these appointments. If you need medication, please make a further appointment with a doctor.
