If your doctor decides you need regular medication, or you are already taking regular medication, they will authorise repeat prescriptions. You will be given a green prescription to take to the chemist, and a white repeat prescription tear-off slip, which you keep. This white slip should not be given to the chemist.
Repeat prescriptions must be ordered in writing using the tear-off slip issued with your prescription. Mark the items that are needed, and cross out any items that are not required. You can either hand the prescription in at reception, through the letter box (if we are closed), or post it to us. Requests can also be faxed to us on 0844 477 3405 (Mitcham Lane) or 0844 477 3406 (Franciscan Road), or e-mailed via our online form.
Please allow 48 hours for your prescription request to be processed.
You can collect your prescription from the surgery during surgery hours, or you can provide a SAE and your prescription will be forwarded onto you.
We will also hand over your prescription to a local chemist of your choice on completion of a consent slip. However, you must re-order your next supply directly from us using the tear-off slip. If you do not have this slip, please call reception to request a copy.
For patient safety, we do not accept prescription requests over the phone.
In line with guidance from the National Patient Safety Agency, Warfarin will only be issued on receipt of a copy of your yellow monitoring book, along with your most recent blood test result. Once we have received this, prescriptions will only be issued before your next blood test is due.
If you are on methotrexate, you will need a blood test every 4-6 weeks. This is either carried out by the hospital responsible for your care, or the GP (if the hospital consultant has agreed to this and has formally transferred your monitoring). Your doctor will then give you a form for your next blood test.
5 days after your blood test, you will need to bring your shared care card and leave it at the surgery. The card will be completed by the GP and your prescription can then be issued.
You will need to make arrangements to collect your card, along with your next blood test form, a few days later.
If you need a prescription for an item that you have had in the past, you will need to put this request in writing. This can be done by adding the item onto the slip, or by completing a request form at reception. All such requests are reviewed by the doctors, and you may be asked to make an appointment to be assessed.
