Patients Charter
The patients charter sets out the standard of service you can expect from Streatham Park Surgery and what we expect from you as a patient.
We agree to:
- Greet you courteously and treat you with respect at all times. We have a policy of equality and diversity
- Make every effort to see you promptly, however, you will be informed of any major delays
- Respect your privacy, dignity and confidentiality
- Give you access to your medical records, subject to any limitations in the law, and keep your computerised medical records under the terms of the Data Protection Act
- Inform you of our services using the practice website, booklet, posters and leaflets
- Allow you to decide whether to take part in research or training
- Have routine appointments with the medical staff available within 2 working days
- Offer you a same-day appointment for urgent medical problems
- Offer you a health check when you first join the
- Offer nurse appointments within 5 working days
- Offer advice to promote health, for example, smoking, exercise, diet and self-help on minor ailments
- Make appropriate and prompt referrals to other health and social care agencies
- Have repeat prescriptions ready to collect within 2 working days
- Inform you of any abnormal results that require further action
- Arrange a home visit if appropriate
In return, we expect you to:
- Treat the staff with courtesy and respect at all times
- Attend appointments on time
- Tell us if you change your name, address and/or telephone number
- Make an appointment for one person only - “one patient, one appointment”
- Make every effort when consulting the surgery to make best use of nursing and medical time
- Give as much notice as possible if you are cancelling an appointment
- Tell us all the details of your past illnesses, medication, hospital admissions, and any other relevant information
- Let us know if you feel there are things you want to see improved or changed in the practice
- Read the practice booklet or visit our website as they contain important information
