Patients Charter

The patients charter sets out the standard of service you can expect from Streatham Park Surgery and what we expect from you as a patient.

We agree to:

  • Greet you courteously and treat you with respect at all times. We have a policy of equality and diversity
  • Make every effort to see you promptly, however, you will be informed of any major delays
  • Respect your privacy, dignity and confidentiality
  • Give you access to your medical records, subject to any limitations in the law, and keep your computerised medical records under the terms of the Data Protection Act
  • Inform you of our services using the practice website, booklet, posters and leaflets
  • Allow you to decide whether to take part in research or training
  • Have routine appointments with the medical staff available within 2 working days
  • Offer you a same-day appointment for urgent medical problems
  • Offer you a health check when you first join the
  • Offer nurse appointments within 5 working days
  • Offer advice to promote health, for example, smoking, exercise, diet and self-help on minor ailments
  • Make appropriate and prompt referrals to other health and social care agencies
  • Have repeat prescriptions ready to collect within 2 working days
  • Inform you of any abnormal results that require further action
  • Arrange a home visit if appropriate
