We currently allow patients to be able to book some appointments up to two weeks in advance. These appointments are between 9am-10am and 4pm-5pm. The remaining appointments (10am-12pm and 5pm-6pm) can be booked on the day.
To book an appointment, you will need to contact the surgery via telephone. Lines are open from 8.30am-1pm and 3.30pm-6pm. Please note the surgery opening times before contacting us.
We now have appointments available on Saturday mornings at both surgeries. These must be booked in advance. Please do not turn up at the surgery expecting to be seen.
Some consultations do not require a face-to-face appointment, and we have the facility to offer a small number of consultations over the phone. Please state whether a telephone consultation is required when booking. The receptionist will take down your phone number and advise on when the doctor is able to ring you back.
Home visits are intended for housebound patients. Although all requests will be assessed by a doctor, other patients may be requested to come into the surgery. Visits are allocated on an individual basis according to medical condition. If a home visit is needed, please ring as soon as possible. Please note that home visits are normally carried out during the afternoon.
If you need medical assistance when the surgery is closed, please telephone the surgery. You will either be transferred to our out-of-hours service, HARMONI, or given instructions on what to do.

Every patient is allocated a 10 minute appointment with a doctor, or a 15 minute appointment with the nurse or health care assistant. To ensure our surgeries run on time, we request that you attend your appointment promptly. If you arrive late for your appointment, you cannot expect to have a full 10 minute consultation.
As part of our aim to reduce patient waiting time, if you arrive more than 10 minutes late for your appointment, you will be asked to re-book the appointment.
It is important that you are aware of Streatham Park Surgery’s policy on failure to attend your routine or registration appointment. If you fail to attend your registration appointment without cancelling, we reserve the right to not offer you another appointment, and you will not be allowed to register with the practice.
If your forms have not been completed upon arrivival, you will have to re-book your appointment. The administration tasks associated with your registration take a minimum of 10 minutes to complete; any delays will impact on other patients.
Please note that, if you fail to keep an on-the-day appointment, or 3 routine appointments within a 12 month period without cancelling, we reserve the right to deduct you from our list.
