We have systems in place for managing chronic health problems like diabetes, heart disease, asthma and blood pressure. Routine monitoring is carried out by the doctors and nurse during medication reviews and intermediary reviews.
There is a nurse-led diabetic and heart disease service. All such patients are offered an annual review. We write to all these patients in the month preceding their review offering an appointment and arranging any appropriate investigations prior to the appointment. The review appointment is 30 minutes long, and includes a detailed physical health check and lifestyle review.
For the elderly, severely mentally ill and patients with complex medical problems, we run a dedicated Medication Review clinic. These are 30 minute appointments with time for managing your medical problems, medication, and any other relevant social or mental health issues.
